(for new students OR updated family information from last year)
Each fall, WAVE publishes a directory of students, staff and administrators. One roster is distributed FREE to each family. Additional copies are available for purchase for $5.00 each.
Please fill out the following form, and note that space is limited.
In our roster each student can have only:
1 - main phone number
1 - address
2 - cell phone numbers (parent 1/parent 2)
2 - email addresses (parent 1/parent 2)
Please note, the school does not use the roster for emergency information, please include guardian information only in our roster. Thank you.
